Alliance for Healthier Communities
Designed to help health care organizations build their capacity around operationalizing health equity, this toolkit maps out resources available in Ontario at the individual user, team, and organizational levels. It includes resources that introduce relevant concepts and describe how these concepts can be applied in practice. The tools are categorized based on a health equity framework adapted from the Institute for Healthcare Improvement’s (2016), Achieving Health Equity: A Guide for Health Care Organizations (Wyatt et al., 2016).
